New Beginnings HCY Program: Transforming At-Home Health Care for Missouri’s Youth

Since 1999, New Beginnings CDS and In-Home Services has been a trusted name in the St. Louis Metropolitan Area, offering compassionate, efficient, and stress-free services to families in need. Their latest initiative, the HCY Program, is designed to provide tailored support for Missouri children and young adults with special health care needs, ensuring they can thrive in the comfort of their homes surrounded by loved ones.

What is the HCY Program?

The HCY (Health Care for Youth) Program provides service coordination and authorization for medically necessary services for MO HealthNet fee-for-service participants from birth to age 21. Aimed at ensuring individuals with special health care needs remain safely in their homes, the program includes comprehensive assessments, home visits, and access to services such as in-home personal care and skilled nursing.

Key services provided include:

Personal care aide support

Private duty nursing

Skilled nursing visits

Advanced personal care for complex health needs

Who is Eligible?

To qualify for the HCY Program, participants must:

Be Missouri residents

Be between birth and age 21

Require medically necessary services

Be enrolled in MO HealthNet (fee-for-service)

Comprehensive Service Coordination

The HCY Program ensures families receive hands-on support at every step. A dedicated service coordinator works closely with participants and their families to:

Evaluate and assess individual needs

Develop and implement service plans tailored to those needs

Identify and connect families with service providers

Support families in accessing resources

Assist in establishing a reliable medical home

Provide guidance for transition planning

This holistic approach ensures children and young adults not only receive high-quality care but also benefit from seamless coordination across various support services.

Call 1-866-835-3505 (press #1 for Pre-Screening) with the participant’s name, Medicaid number, date of birth, and phone number.
